Forms - Legal and otherwise

Here are links to various forms that you might have to fill in:

• The Court Service publishes all the forms that you will need to fill in for the court process, there is an index here.

  Note in particular:

  • Children Act 1989. Most of your contact, etc, forms are here.
  • Divorce forms.

  The ones that you might need are listed below:

• Form C1, this is the form that you use to start a contact case. You may also need to fill in this new form C1A if there are allegations of violence of abuse. Notes to help you fill it in are here.
• If you have an on going case, to start a new case within it you need form C2.
